Privacy Policy

We respect the privacy of our customers and are committed to protecting their information on our websites with the same care we use for all ClearBalance HealthCare® transactions. This document outlines ClearBalance HealthCare’s online privacy practices and applies to anyone who visits our websites (https://clearbalance.org or http://www.myclearbalance.com/ClearPath). It explains how we may collect information from you online when you visit ClearBalance HealthCare websites and how we may protect and use that information. It does not apply to information collected from other sources, including off-line information collection. When we collect financial information from you for applying for or participating in the ClearBalance patient financing program, we will provide you with a financial privacy notice that describes the privacy practices of participating banks, including personal information that we collect for participating banks.

How We Collect Your Information

We collect personal information from you or about you when you provide this information to us directly. For example, we may obtain personal information when you request information regarding products or services from us, register on our websites, respond to surveys, or otherwise interact with us.

How We Use and Share Your Information

We may use and share information collected to:

  • Respond to your inquiries and fulfill your requests;
  • Send you marketing communications that we believe may be of interest to you;
  • Personalize your experience on the Sites by presenting content, ads or offers tailored to you;
  • Allow you to participate in surveys, contests and similar promotions and to administer these activities; for business purposes, including data analysis, audits, developing and improving products and services, enhancing the Sites, identifying usage trends and determining the effectiveness of promotional campaigns.

Legal Proceedings

We will share personal information with third-party companies, organizations, or individuals outside of Company if we have a good-faith belief that access, use, preservation or disclosure of the information is reasonably necessary to:

  • Meet any applicable law, regulation, subpoena, legal process or governmental request.
  • Enforce applicable Terms of Service, including investigation of potential violations.
  • Detect, prevent, or otherwise address fraud, security, or technical issues.
  • Protect against harm to the rights, property or safety of Company, our users, customers or the public as required or permitted by law.

Transfer in the Event of Sale or Change of Control

If the ownership of all or substantially all of our business changes or we otherwise transfer assets relating to our business or the Sites to a third party, such as by merger, acquisition, bankruptcy proceeding or otherwise, we may transfer or sell your personal information to the new owner. In such a case, unless permitted otherwise by applicable law, your information would remain subject to the promises made in the applicable privacy policy, unless you agree differently.

We and certain trusted service providers operating on our behalf collect information about your activity on our websites using automated tracking technologies, including:

Automatically Collected Information

When you visit our websites, some information is automatically collected from you. This information may include, but is not limited to, some or all of the following items: your Internet Protocol (IP) address and/or domain; type and version of Internet browser software and operating system you use; your general location; date, time, and duration of your Site access; specific pages, buttons, images, videos, links, or forms that you access while visiting the Site; type of device (e.g. iPad, smartphone); mobile carrier and/or Internet Service Provider; and demographic information concerning the country of origin of your Computer and the language(s) used by it.


We use Google Analytics, a web analytics service provided by Google to collect certain information relating to your use of the Sites. Google Analytics uses “cookies,” which are text files placed on your computer, to help the websites analyze how users use the sites. The information generated by the cookies about your use of the websites (including your IP address) will be transmitted to and stored by Google on servers in the United States. Google will use this information for the purpose of evaluating your use of the websites, compiling reports on website activity for website operators and providing other services relating to website activity and Internet usage. Google may also transfer this information to third parties where required to do so by law, or where such third parties process the information on Google’s behalf. Google will not associate your IP address with any other data held by Google. You may refuse the use of cookies by selecting the appropriate settings on your browser, however, please note that if you do this you may not be able to use the full functionality of the websites. Information Collected via Cookies

Cookies are small text files that are placed on your computer, tablet or smartphone to distinguish you from other visitors to our Sites. The use of cookies is a standard practice among websites to collect or track information about your activities while using the websites. We and/or our third-party advertising service providers may place cookies or similar files on your hard drive for many of the reasons listed in this Privacy Policy while visiting our sites. Most people do not know that cookies are being placed on their devices when they visit websites because browsers are typically set to accept cookies. You may choose to have your browser warn you every time a cookie is being sent to you or you can choose not to accept cookies. You may also delete cookies from your device at any time. If you refuse cookies, the functionality of our sites may be impacted or become non-functional. Since every browser and electronic device are different, you will need to follow your browser’s instructions for disabling or deleting cookies.

Information Collected via Flash Objects and Other Similar Technology

In addition to using cookies, we and/or our third-party advertising service providers may also use similar technologies to track users’ interactions with our sites. Some of these technologies include web beacons (transparent graphical images placed on a website), pixels, tags, and Flash objects (also referred to as “Local Shared Objects”). Please refer to your browser’s instructions to remove cached Sites, history, and images from your device. Deleting or disabling cookies will not remove Flash objects or Local Shared Objects. Instead refer to Adobe’s website for more information on how to control or disable these items. We may partner with third parties such as Google to deliver advertisements and monitor activities on our own websites and other websites. We do not, however, track your online activities after you leave our sites. We may monitor advertisement “click-throughs,” Web traffic, statistics, and/or other activities in order to improve browsing experiences on our websites.

Security

We use industry standard physical, technical, and administrative security measures and safeguards to protect the confidentiality and security of your personal information. However, since the Internet is not a 100 percent secure environment, we cannot guarantee, ensure, or warrant the security of any information you transmit to us. There is no guarantee that information may not be accessed, disclosed, altered, or destroyed by breach of any of our physical, technical, or managerial safeguards. It is your responsibility to protect the security of your login information. Please note that emails and other communications you may send us via the Contact Us link on our sites are not encrypted, and we strongly advise you not to communicate any confidential information through these means.

Third-Party Websites

Our websites may contain links to third party websites. ClearBalance HealthCare encourages you to be aware when you leave our websites and to read the privacy policies and terms of use of any such websites that may collect your personal information. ClearBalance HealthCare is not responsible for the privacy or security of these websites, including the accuracy, completeness, or reliability of their information.

No Information from Children Under the Age of 13

If you are under the age of 13, please do not attempt to register with us on our websites or provide us with any personal information about yourself. If we learn that we have collected personal information from a child under the age of 13, we will promptly delete that information. If you believe we have collected personal information from a child under the age of 13, please call us at (888) 334-4022.

Email

If you provide your email address to us, or if we obtain it from another source, we may use it to respond or update you on your inquiries or to let you know about products and services that may be available to you. If we send you an email advertisement, it will include instructions on how to opt out of receiving such emails in the future. If you elect to opt out of receiving email advertisements, we may still send you emails about your account relationships with us. If your name was previously selected for any of our upcoming email offers, it is possible for you to receive information from ClearBalance HealthCare up to 15 business days after your request has been submitted.

International Privacy Practices

Our sites are operated and managed on servers located and operated within the United States. By using and accessing our sites, residents and citizens of countries and jurisdictions outside of the United States agree and consent to the transfer and processing of personal information on servers located in the United States, and that the protection of such information may be different than required under the laws of your residence or location.

Changes to Our Privacy Policy

Our Privacy Policy may change from time to time. We will not reduce your rights under this Privacy Policy without your consent in accordance with applicable law. We will post any privacy policy changes on this page and, if the changes are significant, we will provide a more prominent notice (including, for certain services, email notification of privacy policy changes). We will also keep prior versions of this Privacy Policy in an archive for your review.
